Each staff person is being asked to complete a program-evaluation form. This is not a staff performance form. You are rating the performance of the program overall. There are to be no names or identifying marks attached to this form and no names of individual staff should be included in your comments. We are interested in the substance of your thinking in order to create improvement, not to punish or reward. Leave blank any area you believe you cannot answer. Score each section as indicated. You will note that we are interested in your own personal beliefs about the students and the program. Again, this is not a test, by a needs assessment. You do not need to agree with your peers as to what the issues are. We want your personal perspective.
